ACORD Forms

Standardized insurance forms and data standards maintained by ACORD and used industry-wide for applications, certificates and data exchange.

ACORD (the Association for Cooperative Operations Research and Development) is the nonprofit standards body for the insurance industry. ACORD forms are the standardized paper and electronic documents agencies use so information means the same thing across carriers.

Familiar examples include the ACORD 125/126/140 commercial applications, the ACORD 80/HO application for homeowners, and the ACORD 25 certificate of liability insurance.

  • Forms standardize how information is collected and presented.
  • Data standards (ACORD XML/AL3) let systems exchange that information, underpinning download and real-time.

Because ACORD forms are recognized industry-wide, they reduce ambiguity and are a core part of E&O documentation.
